These cells, which are part of the immune system, can overgrow and crowd out the normal blood-forming cells, leading to a range of symptoms and complications. Understanding Multiple Myeloma
What is Multiple Myeloma?: Multiple myeloma is a cancer that starts in plasma cells, a type of white blood cells found in the bone marrow. Normal plasma cells help protect the body from infections and other harmful substances by producing antibodies. However, in multiple myeloma, these cells start to multiply uncontrollably, leading to a surplus of abnormal plasma cells in the bone marrow.
Effects on the Body: This overgrowth of plasma cells compresses the normal bone marrow, leading to a decrease in the production of normal blood cells. This can result in anemia, frequent infections, and a weakened immune system. The multiple myeloma cells themselves can also secrete substances that weaken the bones, making them more susceptible to fractures. Additionally, they can cause kidney damage and failure due to their harmful antibodies.

Treatment Options for Multiple Myeloma
Chemotherapy and Radiotherapy: One of the primary treatment methods for multiple myeloma is chemotherapy, which involves the use of drugs to kill cancer cells. For example, some steroids are often used in combination with chemotherapy to enhance its effectiveness. Another effective drug is Thalidomide, which has shown efficacy in treating myeloid leukemia but comes with risks, such as causing birth defects and increasing the risk of blood clots.
Combination Therapy: In some cases, particularly in fit patients in the United States, doctors combine several chemotherapy agents with an autologous stem cell transplant. This approach often leads to better outcomes, and it is one of the areas where there has been significant progress in recent years with the FDA approving several new medications.
Recent Advancements in Treatment
Immunomodulatory Drugs: Combining the use of immunomodulatory drugs like Lenalidomide and Pomalidomide with drugs such as proteasome inhibitors and corticosteroids can induce remissions in most patients. However, it is important to note that almost all patients eventually relapse due to developed resistance to these drugs.
Proteomics and Relapse Mechanisms: Recent research has shed light on the mechanisms behind the relapse of multiple myeloma. An interdisciplinary team from Berlin has identified CDK6 upregulation as a non-genetic resistance mechanism for immunomodulatory drugs in multiple myeloma. This discovery could pave the way for new treatment strategies to overcome drug resistance.

Practical Treatment Strategies: Depending on the stage and severity of the disease, treatments can range from radiation for bone pain, surgery in oncology emergencies, blood transfusions, dialysis for kidney failure, and plasma exchange.
