Personal Experience: CPAP Use and Residual Daytime Sleepiness
I have used a CPAP for 12 years for severe sleep apnea. Unfortunately, I also have periodic limb movement disorder (PLMD) which wakes me up repeatedly at night. The CPAP helps, but I do feel moderately more tired and have dry mouth on those rare occasions when I do not use it. However, I have never experienced the 'wow, it's a miracle; suddenly, I feel well rested!' response. Despite this, I have used the CPAP faithfully since both of my parents died of heart disease at a young age and I have my own hypertension issues.

After my first two years of CPAP use, I was still very fatigued each day and incredibly frustrated. Based on multiple tests, the doctor noted that my PLMD was still waking me up and contributing to sleepiness during the day. I was tested for narcolepsy, but the results were negative, and I was diagnosed with 'idiopathic hypersomnia.' I was on iron supplements for years and tried various medications such as gabapentin, Requip, and Klonopin to help reduce the nightly leg movement that caused arousals. However, these had little effect. The daytime sleepiness was significantly affecting my work and overall well-being, so eventually, I was prescribed Modafinil. It made a huge difference and greatly improved my ability to work the long hours needed as an attorney.
Modafinil: An Accompanying Treatment for Sleep Apnea
It is essential to note that Modafinil is not intended to replace CPAP therapy or other treatments for sleep apnea. While CPAP is the primary and most effective treatment for OSA, Modafinil is prescribed to manage the residual daytime sleepiness that persists despite the use of CPAP.
